So, do you guys have a notion of what setting might affect this? I’ve been unable to find it so far!
Two rules when you want to understand your files: turn voice colours on (not relevant here) and signposts visible (VERY relevant here).
You could use the Library Manager, compare to factory settings, and reset note and beam groupings under notation options.

Was this in a project you started from scratch or is it an XML import?
OK guys - I found the setting following Jesper’s instructions (for those interested!). The check marked setting is correct - for some reason I had changed it.
I’m still confident that the example you posted in the OP in 12/8 is not what Dorico would produce without manual intervention (I’m specifically talking about the half note that follows the two 16th notes in the second bar). This notation is totally wrong and Dorico wouldn’t produce it on its own.
You are correct - I had changed this setting, not really understanding the implications. Now I know!
It’s weird though that that setting creates a beat grouping in 12/8 that is so flat out wrong. I can’t believe that setting is intended to create results like these in a compound time signature.
I suspect the setting is actually meant for 4/4 time sigs only. But it has a mathematical impact on compound time signatures as well.
