It seems the line tools do not use a "musical position → sample position " conversion routine for determining the position of a new breakpoint.
The tool is always using sample positions, not the positions as per the “Primary Time Format” or as per the main ruler setting.
This seems to happen in both cases - VST automation and MIDI messages.
