Faders move program volume based on index number, not MIDI

I am using Cubase 6.0.3 64-bit, Windows 7 64-bit, and HALion 4.0.1 64-bit.

There is a problem with how the MIDI channel faders in the Cubase Mixer are assigned to HALion 4 volume controls. When I move the fader assigned to MIDI channel 4, it should move the HALion 4 volume level for the program slot assigned to MIDI channel 4. But it does not! It moves the HALion 4 volume level for the program slot assigned to Index number 4 instead. (By Index number I mean the little number on the upper-left corner of the instrument pictures in the Slot Rack.)

As a workaround I make sure the Index numbers match the MIDI channels. But could I have something configured incorrectly? Is this a bug?

Also, is there a way to re-index based on MIDI value or the order the slots are displayed in the rack?
