My MR816x developed a fairly loud hum.
Bottom line - CH1 set to HI-Z, and phantom power on CH1 (+48V), hum, even with nothing plugged into the channel.
Now, HI-Z and phantom power is not a combination that has any valid application, so it would likely be set inadvertently.
