When setting the Orientation of Chord Diagrams to “Horizontal” in Engraving Options, the Design option #11 (Distance from chord symbol) isn’t having any effect for me. I can set it to “0”, or “24”, and the chord symbol and diagram remain at the same relative distance.
It works for me. In case it made a difference, I even went so far as to change all the values in Design options 4 to 11 to those shown in your screenshot. When I altered the value in design option 11, the change was reflected in both the Full Score layout and the Guitar layout. It worked with Chord diagram preset and/or Orientation set to Horizontal (which yours is) and also Vertical. As is normal with any of the Options, the change did not happen in the layouts until I clicked on Apply. One thing I did notice is that the orientation needs to be set to Horizontal or Vertical before setting the Distance from chord symbol.
In case there is another option or setting which is influencing things, click on Reset to Factory in Engraving Options, then change Option 11 again and see if that has any effect.
If this recalcitrance persists, the best way to let others investigate is to post a file (or a portion thereof) which exhibits the problem.

I have tried it in Dorico 6, 5, and 4. It worked OK for me in all of those. Maybe it is an intermittent problem, perhaps affected by particular settings/options. I tried changing every option I could find which related to chord diagrams to see if they had any effect, but I was still able to change the distance. The only odd thing I noticed is that when I changed the value for Distance from chord symbol, the Tempo marking moved the same distance as the Chord Symbol.
