Can anyone suggest why the part ‘Andy’ is missing the second flow title?

Because Andy’s first page has an override, indicated by its red corner in the right panel of Engrave mode. Remove it and the flow title appears.
If you look carefully, you can actually see that the override was created when somebody dragged the Flow Heading down to between the second and third systems of that second flow, and then deleted its contents.
