After changing certain options in Notation Options and clicking “Apply” and “Save as Default”, then, when I crate a new document, I notice that the options returned to their original state. Do I have to change these everything I create a new document?
No, you should find that they are applied when starting a new project. Check that you have the expected file notationOptions.xml in ~/Library/Application Support/Steinberg/Dorico, which contains the saved settings you have changed.

I checked and I do have the notationOptions.xml.

After saving as default (Saves as default.png)), this is what I see(Original default.png) when created a new project.
Thanks for following up on this, Nicholas. It looks as if there’s a bug here: the options are being saved correctly, but they’re not being applied as expected. We’ll take a look at this in the New Year and try to resolve it as quickly as we can.
