Somehow, I’ve deleted the default quarter note notehead. How can I re-add it?

Can you post your file here, or a cut down version that demonstrates the issue? I can think of a couple of different possibilities (most likely editing the notehead set, but possibly also editing music symbols).

This trips a lot of users up. When you do New From Selection in the notehead set editor, you are creating a new set which contains the same noteheads. Any edits you make to those noteheads are also reflected in other sets which contain them.

In your case, since you want a set with just a single notehead, it’s a little easier to just do New instead of New From Selection – but there’s still the same issue, since the new set starts out with the default noteheadBlack.

How to customize note heads in Dorico?

As I indicated, this is a very confusing part of Dorico. Notehead sets have to contain at least one notehead, so when you create a new one, it includes the default “noteheadBlack” notehead, and if you edit that, your changes will also affect that notehead in the “Default Noteheads” set. And just so you know, you can get parentheses around a notehead without having to create a new notehead set. Just select the note in the score, open the bottom Properties panel, scroll over to the Bracketed Noteheads section, and activate the Bracket Style toggle.
