Since the latest update of D.4 I’ve been having a universal problem with tied grace notes repeating again later in the beat. It happens in all scores that I’ve been working on. I’m attaching a silent clip so I don’t know if it will work or not. I was going to attach a sound clip but it doesn’t happen there.

I don’t think there’s anything untoward going on here. An unslashed grace note like that will be played as an appoggiatura by default, i.e. it will be played on the beat and at half the duration of the rhythmic note it follows. That is clearly not what you want, so go to the Timing page of Playback Options and set Maximum length for unslashed grace notes to play back as short appoggiatura to the eighth note value. Or use a slashed grace note, if you prefer.
