I admit I’m guessing about the oversized note heads, but it seems like the sort of thing where computer software does what you tell it to do literally, not by using “common sense.”
There are two settings in Engraving Options for the minimum distance above the top staff line and the minimum separation from objects above the staff. Probably if you set them both the same the two 4’s would line up (or as near as makes no difference).
