After many pages of correct open meter, suddenly voices don’t align. See snippet. I don’t believe fudging it all in Engrave Mode is the answer. What has gone wrong? What could go wrong? After this bit the next page shows a gross misalignment between staves also. What could possibly cause this? Anybody seen this before?
Should I post the whole score?
Maybe somewhere a small hidden rest?
I thinks that would make it easier 
Or just a small part of it, as long as it shows the problem.
By cutting down to the bare minimum, you might find the cause yourself
But why? This has only just started happening after much note entry..

Have you Force Duration locked? That could prevent you to notice that you’re entering a note at the wrong place. (Just an idea…)
Sorry, since you’re in open meter, I think my remark is irrelevent.
Here is the complete score. It’s open meter for several pages at the end. You can see that the rests in the last page dont line up either and the staves seem to have different number of beats (see the very end).

If you turn the F#D# rh chord into a 32nd (with insert mode), then at least the beats of left hand and right hand are aligned, I think:
And also the Eb (it was still offset in the screenshot above just because of the voice column index). It’s now at the same rhythmic position as the F:
The RH upstem voice 1 was offset by a 64th because of the dotted 32d (F#D#), which should have been an undotted 32d.
You must also include an eighth‑note value in the left hand so that the implicit rests begin at the same position.
