“Baritone (in score)” and “Baritone Horn in (score)”.
They both belong to the brass family and look the same.
Could someone please explain the difference ?
There is no difference. Baritone is an alias for Baritone Horn as it is often shortened to just Baritone.
Baritone/euphonium player here. They are indeed the same, though I don’t recall ever seeing it being called Baritone Horn in a score or part before. I’d personally consider the presence of Baritone Horn in Dorico like the French Horn vs. Horn thing, where French Horn (as Baritone Horn) is a name you’ll hardly ever see in sheet music.
