You left me and @ecou a message on the LoopyPro forum this morning concerning the topic title, I will keep you informed in this forum immediately if the issue continues after the next Cubasis 3 update.
As I explained in my message to @ecou, I have reported this problem a long while ago and because no one else was having this trouble that I knew about, I investigated and found what was causing the occasional note not being triggered and I was able delete that anomaly as I have marked in my screenshot below. This occasionally happens when using the CUT tool to slice a note or chord into smaller note values and unless one knows where to look this issue continues to be a major problem.
