What is the convention for numbered bar regions and bar repeat regions in piano music? Should the numbers only appear over the RH staff?
I’d say that for numbered bar regions, above the RH staff only is the convention.
Here for example in Bartók’s Miraculous Mandarin (Boosey & Hawkes, piano part)
Thanks for the example. And I’m guessing it’s probably the same for bar repeats, though you wouldn’t see that in the classical rep.
Or would it be better in this case to (manually) center the counts?
That’s why I didn’t comment on that point — it’s not my area, so I prefer to leave it to those who are used to this kind of notation. That being said, the two examples you show look fine to me (the second maybe more elegant? But if there’s no option for that, I’m not sure it’s really worth it!)

FWIW, I went with bar repeat numbers centered between the staves. One of the pictures in Gould shows this style (p. 578), although she doesn’t explicitly state it as a rule, and it seems clearer than having the number above.
As a feature-request, it would be nice if Dorico offered this option natively for bar repeats in grand staff instruments.
