This only started happening in D 4.2. and has happened 3 or 4 times. As can be seen in the first pic in the next to the last bar, bottom stave there is what appears to be a quarter note followed by a triplet with a missing beat. Took me a while to figure out that the quarter note is actually the middle note of the triplet that has been displaced. I can straighten it out using the horizontal spacing tool but am wondering how something like this can happen at all?
