In Engraving Options–>Tablature–>Stems, I have “Stem End Position” set to “Outermost note”. This works unless I have a half note or whole note in one voice on the same beat as a note in the other voice, in which case Dorico inserts a stem through to the innermost note (between the two voices).
Is this a bug or am I doing something wrong?
Could you attach a screenshot, or better yet a small project, as I’m not sure I follow the description of the problem.

I have basically nil experience with tablature, but being inquisitive I thought I would have a look at your file. I was getting nowhere until I did Reset to Factory in Engraving Options. That removed the extra stem length, even after I re-applied Outermost note. It also removed the circle around the lower note on the first beat of bar 5. This leads me to think that there might have been some changes you made to the notehead set(s), specifically to half notes and whole notes. Others with more experience will be able to make more informed comments.
In Library > Library Manager, I selected Engraving Options from the Options list on the left, clicked on Show: Differences, then Tablature. Enclosures for notes and chords longer than a quarter note (crotchet) showed that Rounded rectangles was different from factory settings.
Clicking on the red ≠ symbol (which then turned to a green arrow) and clicking on Apply changed the appearance of bar 5 to this:
Hopefully this information will give you some ideas on what to do next.
The following two screenshots speak for themselves.
They might give @Richard_Lanyon some clues as to whether there is something which is not quite as it should be.
